What is After Care

After care is a follow-up support that aims to fill a service gap experienced by many.

After care plans are developed based on the individual and their community. They will aid clients in navigating and utilizing existing service providers and will advocate for clients that may need extra assistance.

After Care Services

After care connects and supports people by using internal and external resources available in each community. After care will be offered in all communities throughout the Beaufort Delta Region.

The After Care Coordinator, based in Inuvik, will:

  • Help clients develop after care plans
  • Connect with clients on-the-land
  • Make community visits
  • Communicate by telephone and email to ensure adequate support is given
  • Navigate community resources (government and non-government)
  • Advocate for clients
  • Provide case management
